Butler County Health Care Center is pleased to announce their new patient portal, My Butler County Health.  The patient portal is a convenient and secure website that enables Butler County Health Care Center patients and family members to access, view and print their personal health records.  All of the information is gathered from the patient’s electronic health record at Butler County Health Care Center in order to ensure the most accurate and current information possible. 

 

My Butler County Health gives patients access to their health information  including: laboratory and radiology results, medication list prescribed while at Butler County Health Care Center, allergies, upcoming appointments at the hospital and demographic information.  In addition to accessing patient health information, family members and representatives with authorized consent are able to access the records of those individuals for whom they manage care.

 

There are two options for enrollment for My Butler County Health, self-enrollment or enrollment at Butler County Health Care Center.  Self-enrollment is available for those who have been a patient at Butler County Health Care Center and who have given a valid email address to the hospital at the time of admission.  To self-enroll visit: www.bchccnet.org and click the My Butler County Health Self-Enrollment tab or visit enroll.mybutlercountyhealth.net.   Fill in the required information and click the submit button.  Please note you must have your medical record number to self-enroll, which can be found on your patient dismissal instructions.  Once all the information has been processed you will receive an email with a one-time user name, password and access link.  Click this link and insert the required information.  You will then enter your new user name and password and are able to view your electronic health records.

 

The other option is to sign up with a representative at Butler County Health Care Center.  If you do not have an email address, we will assist you in obtaining one.  A short My Butler County Health tutorial will be provided at this time.  In order to access other individuals’ records you must register at Butler County Health Care Center and have a signed proxy form from the individual(s).
