Mobile App Privacy Policy

Introduction

Butler County Health ("BCH" or "We") respects your privacy and is committed to protecting it through our compliance with this policy. This policy describes: 

  • The types of information we may collect or that you may provide when you download, create an account on, access, or use the Wellness Connect mobile application (the "App").
  • Our practices for collecting, using, maintaining, protecting, and disclosing that information.

This policy applies only to information we collect in this App and in email, text, and other electronic communications sent through or in connection with this App. 

This policy DOES NOT apply to information that:

  • We collect offline or on any other BCH apps or websites, including websites you may access through this App. 
  • You provide to or is collected by any third party, including other apps, websites, or content that may link to or be accessible from or through this App.

Our websites and apps, and these other third party websites and apps, have their own privacy policies, which we encourage you to read before providing information on or through them.

Please read this policy carefully to understand our policies and practices regarding your information and how we will treat it. If you do not agree with our policies and practices, do not download, create an account, or use this App. By downloading, creating an account, or using this App, you agree to this privacy policy. This policy may change from time to time (see Changes to Our Privacy Policy). Your continued use of this App after we revise this policy means you accept those changes, so please check the policy periodically for updates. 

Information We Collect and How We Collect It

We collect information from and about users of the App:

  • Directly from you when you provide it to us.
  • Automatically when you use the App. 

Information You Provide to Us

When you download, create an account, or use this App, we may ask you provide information by which you may be personally identified (“personal information”) that includes:

  • Contact Information – this includes information such as name, phone number, and email address. 
  • Other Information – this includes information such as date of birth.
  • Health & Fitness Information – this includes information such as activity and fitness class participation data.

This is information is collected when: 

  • You fill in forms in the App. This includes information provided at the time of creating an account on the App. We may also ask you for information when you report a problem with the App.

Information We Collect Through Automatic Data Collection Technologies   

As you navigate through and interact with the App, including when you download, access, or use the App, we may use automatic data collection technologies to collect certain information, which may constitute personal information, about your device and your browsing and usage activity and patterns, including:

  • Precise Location Information – this includes real-time information about the location of your device to support a core feature that detects when a user is near a facility location and enables a seamless check-in experience by prompting access to their access card barcode. Location data is processed on device to trigger relevant notifications and is not stored by the App, nor is it stored on our or our service provider’s servers or shared with any third parties beyond our App service provider. This precise location information will only be collected with your permission, and you can manage or disable location permissions at any time through their device settings.

How We Use Your Information

We use information that we collect about you or that you provide to us, including any personal information, to:

  • Provide you with the App and its contents, and any other information, products, or services that you request from us.
  • Fulfill any other purpose for which you provide the information.

The usage, identifier, and diagnostic information we collect helps us to improve the App and to deliver a better and more personalized experience by enabling us to:

  • Estimate our audience size and usage patterns.
  • Store information about your use of the App. 
  • Address outages or performance issues with the App.

Disclosure of Your Information

We may disclose aggregated information about our users, and information that does not identify any individual without restriction. 

In addition, we may disclose personal information that we collect or you provide:

  • To our subsidiaries and affiliates.
  • To contractors, service providers, and other third parties we use to support the App and our business.
  • To a buyer or other successor in the event of a merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of the BCH’s assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which personal information held by the BCH about the App users is among the assets transferred.
  • To fulfill the purpose for which you provide it. 
  • For any other purpose disclosed by us when you provide the information.
  • With your consent.

We may also disclose your personal information:

  • To comply with any court order, law, or legal process, including to respond to any government or regulatory request.
  • If we believe disclosure is necessary or appropriate to protect the rights, property, or safety of the BCH, our customers, or others.

All personal information identified herein is retained as long as necessary to achieve the purpose for which the data was collected, or as long as necessary to comply with or as permitted by applicable law.

Choices About Our Collection, Use, and Disclosure of Your Information

We strive to provide you with choices regarding the personal information you provide to us. This section describes mechanisms we provide for you to control certain uses and disclosures of over your information. 

  • Location Information. As described above, in the Information We Collect section, you can choose whether or not to allow the App to collect and use real-time information about your device's location through the device's settings. If you block the use of location information, some parts of the App may become inaccessible or not function properly.

We do not control third parties' collection or use of your information to serve interest-based advertising. However, these third parties may provide you with ways to choose not to have your information collected or used in this way. You can opt out of receiving targeted ads from members of the Network Advertising Initiative ("NAI") on the NAI's website.

Accessing and Correcting Your Personal Information

You may also send us an email at wellnesscenter@bchccnet.org to request access to, correct, or delete any personal information that you have provided to us. We cannot delete your personal information except by also deleting your user account. We may not accommodate a request to change information if we believe the change would violate any law or legal requirement or cause the information to be incorrect. 

Data Security

We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure. 

The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of the App, you are responsible for keeping this password confidential. We ask you not to share your password with anyone. 

Unfortunately, the transmission of information via the internet and mobile platforms is not completely secure. Although we deploy measures to protect your personal information, we cannot guarantee the security of your personal information transmitted through the App. We are not responsible for circumvention of any privacy settings or security measures included through the App.

Children Under the Age of 13

The App is not intended for children under the age of 13 years of age. No one under age 13 may provide any information from children under 13. We do not knowingly collect personal information from children under 13. If you are under 13, do not use or provide any information on this App.  If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. Changes to Our Privacy Policy

We may update our privacy policy from time to time. If we make material changes to how we treat our users' personal information, it is our policy to notify you through via the email address you have provided to the App.

The date the privacy policy was last revised is identified at the top of the page. You are responsible for ensuring we have an up-to-date active and deliverable email address for you and for periodically visiting this privacy policy to check for any changes.

The Parkinson's Empowerment Group at Butler County Health targets so many aspects and symptoms of Parkinson’s. It improves a person’s posture, mobility, balance, and coordination, as well as cognition and ability to multi-task. I also like the flexibility of the program and being able to adjust exercises from each position to fit the need of each individual patient.

BCH Roots to Shoots Pediatric Therapy Clinic eliminates the need for long commutes to distant facilities like Madonna, allowing Aria to maximize her therapy sessions and educational opportunities.  It is significant for Aria’s overall well-being as it reduces the stress and agitation she had from extended travel.  The staff is like our family to us.

I feel having a 3D mammogram was a blessing for me since it detected my Stage 1 cancer.  The technicians were very helpful as they explained how the machine worked compared to the 2D method.  A big difference is that the 3D machines moves above your head from one size to another and gives a more complete image of the breast.  The testing time also seemed shorter than the standard mammogram.

I had a bunion on my foot and I wanted it removed,  but I really didn't want to travel too far to have the procedure done.  So when Butler County Health started providing Podiatry Services, I made an appointment right away.  Dr. Carlston is professional and personable.  He took the time to explain what happens during the procedure and afterwards.
